Used to gather current health information on campers (while designed specifically for campers, this form can be used for staff) contact information, allergies, medications, immunizations, and permission to treat. Form 1 does not require a physician's signature. For resident/overnight camps, the four-page Camper Health History Form 1 is accompanied by a one-page, stand alone, Camper Health-Care Recommendations by Licensed Medical Personnel Form 2.
Day Camps need only use Form 1 to meet ACA-accreditation standards (HW-2). These two forms used together will meet the ACA-accreditation standards for Health History and Health Exams (HW-2 and HW-6) for resident/overnight camps.
